  It was HORRIBL. I filled out the form online for my daughter who is going to be seventeen in a 2 months and was not informed that she could not have the surgery. I had to reschedule 2 times because of work related issues so they obviously looked at the file more than once. Then I drove for an hour both ways and was only there for 5 minutes because when they asked her age they said they would not even do anything. Even if I were to sign a form of consent. By far a major waste of time because my daughter NEEDS this is order to be a fighter pilot in the airforce and now she is completely set back because of incompetent people in a huge office with only 2 people on the job.
